They are seeking a dedicated Post-Release Service Intensive Level 3 Case Manager to provide comprehensive case management services to unaccompanied alien children who have been released from federal custody, ensuring their safety and stability in the community.

Responsibilities

Conduct weekly and then monthly in-person, thorough assessments of each unaccompanied UAC's needs, strengths, and challenges
Be prepared to respond to crises and emergencies affecting UAC, working in collaboration with emergency services and appropriate agencies as necessary
Provide crisis-intervention and ongoing case management services to, including assistance with accessing education, medical care, mental health services, legal assistance, and other essential resources
Conduct UAC centered needs assessments, develops individualized strength-based, goal-oriented service plans, and monitors progress of goal completion
Provides ongoing case management with a focus on screening and making appropriate referrals to address UAC's needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, UAC enrichment activities
Serve as an advocate for the rights and well-being of UAC’s, ensuring that they receive fair and appropriate treatment within the community and addressing any issues or concerns promptly
Conduct outreach and education to community service providers to supplement existing services that do not adequately address unique needs of immigrant UAC
Will coordinate referrals and address barriers in relevant service need areas such as legal, education, medical, mental health; supports unaccompanied UAC and sponsor with enrollment and access to services in community
Assist UAC in enrolling in educational programs, accessing tutoring, and developing life skills essential for self-sufficiency
Demonstrate cultural sensitivity and understanding when working with UAC from diverse backgrounds, including immigrants and refugees
Maintain accurate and up-to-date case records, progress notes, and reports, ensuring compliance with all legal and regulatory requirements
Collaborate with local organizations, service providers, and community resources to enhance the support network available to UACs
Work closely with colleagues, including social workers, counselors, and legal professionals, to provide comprehensive care and support to UAC’s
Stay informed about changes in immigration policies, laws, and regulations, and participate in relevant training and professional development activities
Frequent local travel such as commuting no more than 120 miles each way to a client’s residence from the PRS Intensive Level 3 Case Manager residence in their personal vehicle is required

Required

Master's in social work, psychology, sociology, or other behavioral science in which direct clinical experience is a program requirement, or Bachelor's and minimum 5 years of clinical experience
Must possess knowledge of community social services and working with the refugee populations
Bilingual in English and Spanish required
A valid California driver's license, proof of current automobile insurance

Preferred

Licensed or eligible for licensure preferred
2 years in case management within UAC welfare programs preferably with immigrant or refugee populations
