Care Navigator - PMC Infectious Disease Maps

Responsibilities
Conduct an accurate patient-centered, cultural, sex, and gender-sensitive risk assessment with clients and thoroughly explain the importance of engagement in care and achieving cure for infectious diseases
Provide emotional support, counseling, and advocacy for patients throughout the process of treatment for infectious diseases
Facilitate and ensure that all patients receive the standard of care associated with infectious diseases, regardless of the location of their treatment
Assist patients, families, other caregivers, and facilities in coordinating the treatment of infectious diseases for patients
Maintain relationships with facilities who partner with Infectious Diseases
Work on a multi-disciplinary team of physicians, nurses, pharmacists, social workers, and other members of the healthcare system
Maintain documentation in the Electronic Medical Record as well as spreadsheets used to coordinate care of patients and track outcomes
Assist with benefits counseling including assessing for state coverage, patient assistance programs, copay assistance, and navigating insurance barriers
Qualification
Required
Bachelor of Arts or Science (Required)
Education Specialization: Social Work, Public Health or related field Equivalent Experience
2+ years of experience
Master of Arts or Science
Education Specialization: Social Work, Public Health or related field Equivalent Experience
