Pharmacist (Facility Program Manager- CMOP)
The Tennessee Valley HCS is seeking a Facility Program Manager for their pharmacy services. This role involves providing a broad range of professional and clinical pharmacy services, managing the pharmacy fee/non-VA community care prescription program, and ensuring compliance with the National Formulary and prior authorization drug requests.

Responsibilities
Ensure efficient and timely workload management of the local prescription pending file and act as a liaison and resolve problems as they arise with this program
Knowledge of the CPRS package, outpatient pharmacy current version, and the drug files associated with these pharmacy packages
Requires knowledge of and active participation in all the duties and responsibilities of a staff pharmacist position in the Outpatient Section of Pharmacy Service as assigned
Provides patient care that is appropriate to the cognitive, emotional, and chronological maturation needs of the adult and/or geriatric patient
Conduct drug therapy monitoring in all areas of pharmacy practice as an ongoing activity to include: The therapeutic appropriateness of the patient's drug regimen, dose, frequency, and route/method of administration. Real or potential allergies or sensitivities. Therapeutic duplication in the patient's drug regimen. The degree of patient adherence to the drug regimen. Drug/drug, drug/food, drug/laboratory, or drug/disease interactions. Clinical and laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects
Detecting, monitoring and reporting adverse drug events and medication errors
Participates in conducting MUE studies and departmental quality assurance activities
Evaluates medication orders and medication profiles for appropriateness of therapy; dosage range, synergistic, antagonistic, and cumulative effects. Reviews questionable medication orders with the prescribing physicians when necessary
Provides consultation and education to physician, nurses and other health care professionals on drug information
Ensures that the patient is able to understand medication instruction by providing oral and written counseling on his/her drug therapy
Evaluates physician's orders as to appropriateness based on patient medication profiles; inputs information into computer system; dispenses medication in accordance with appropriate formulary requirements
Qualification
Required
U.S. Citizenship
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Proficient in spoken and written English
1 year of experience equivalent to the next lower grade level (GS-12)
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
