They are seeking Clinical Pharmacists to join specialized teams in Surgery, Neurology, and Internal Medicine to provide comprehensive medication management and participate in interdisciplinary care.

Responsibilities

Deliver direct patient care through medication management
Participate in interdisciplinary rounds and clinical decision-making
Educate patients and providers on pharmacotherapy
Contribute to quality improvement and protocol development
Provides clinical pharmaceutical care to patients
Enhances the quality of delivery of care
Demonstrates proficiency in Pharmacy distributive services
Supports the educational and professional development of the Pharmacy department
Engages in professional self-development

Qualification

Pharmacy DegreePGY-1 ResidencyVirginia Pharmacy LicenseBLS CertificationPatient EducationInterdisciplinary CollaborationAttention to Detail

Required

Bachelor's in Pharmacy or Pharm. D Degree required
PGY-1 residency required or equivalent hospital experience. (3 years)
Commonwealth of Virginia State Board of Pharmacy. License required
American Heart Association (AHA) Health Care Provider. BLS certification required

Preferred

PGY-2 residency is preferred for specialty areas
