Responsibilities
Design timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management
Evaluate and analyze health care plan and resources
Tracks client progress towards goals, modifying care plans as needed
Communicate with treating facilities to coordinate patient care
Participate in clinical audits and performance reviews to advance patient safety and care effectiveness
Help patients navigate healthcare systems and access necessary resources
Qualification
Required
Bachelor's or graduate/higher level degree: major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position
One year of specialized experience which includes collaborating with healthcare professionals, communicating with patients, and researching healthcare resources/programs
Possess a valid operator's license in order to drive a motor vehicle to and from work sites
Preferred
Developing and implementing programs to reduce the risk of suicide
Providing direct support to at-risk individuals
Coordinating between various resources
