Behavioral Health Case Manager

Responsibilities
Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients
Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions
Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care
Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements
Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care
Qualification
Required
Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position
One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care
Must be able to obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles
Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation
A Personnel Security Investigation may be required
Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 315
This position is a Testing Designated Position subject to pre-employment screening and random drug testing
Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk
This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination
This position may require rotating shifts
