Pharmacist (Clinical Specialist)- Community Living Center Long Term Care

Responsibilities

Routine review of medical orders for appropriateness of the drug, dosage, schedule, duration, and directions
Routine polypharmacy reviews to identify opportunities to reduce medication and therapeutic duplications and enhance patient medication administration compliance
Routine formulation of clinical medication therapy management decisions based on a review of the patients' computerized medication profile, remote computerized patient record system, patient consultations, and provider consultations. Decision analysis may include obtaining a patient medication history and reviewing the medication profile for prescription duplication, potential drug-drug interactions, drug-nutrient interactions, therapeutic duplication, prescription noncompliance, laboratory reports, and allergies / adverse drug reactions
Routine provision of medication and therapeutic consultations for assigned patients
Routine review to monitor and ensure patient compliance and management outcomes with regard to medication therapy. Monitoring activities may include the ordering of specific lab tests
Coordinate and provide direct disease state management as a mid-level provider
Routine documentation of clinical and operational activities as assigned
Actively support the department's medication and fiscal management activities and programs
Routine support for medication formulary compliance, medication conversions, therapy treatment and pathway compliance, and drug utilization evaluations as assigned
Participate in the design, implementation, and reporting of clinical and pharmacoeconomic studies and evaluations as assigned
Objectively support adherence to the formulary and demonstrate the assertiveness and poise that is needed to handle any untoward situations. Support the review and adjudication of non-formulary medication orders consistent with departmental guidelines / policy
Support the medication-related education of medical and non-medical residents, interns, and students as assigned
Evaluate and synthesize information from primary and secondary sources of clinical drug literature as needed to support clinical decisions and patient / provider education
Actively participate in all educational programs of the Clinical Pharmacy Section and any staff development lectures
Provide direct medication related education for patients

Required

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Physical Requirements: See VA Directive and Handbook 5019
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
Must be proficient in written and spoken English
All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment
Selective Service Registration is required for males born after 12/31/1959

Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level (GS-12)
Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
