Responsibilities

Uses established guidelines to evaluate and determine eligibility for public health/nutrition programs; conducts health screenings; uses educational materials; implements educational plans; provides preventive health education; evaluates individuals, families and/or groups for other health, economic and social needs; maintains resource referral information; provides referral of client families/groups to other social programs; conducts directed outreach to increase caseload; ensures accurate documentation and recordkeeping

Uses established guidelines to evaluate and determine eligibility for public health programs; develops, selects, and revises educational materials; develops and implements educational plans; provides preventive health education; develops and maintains referral and outreach network; provides referral of client families/groups to other social programs; conducts marketing and community outreach to increase services; ensures accurate documentation and recordkeeping; prepares media articles; completes administrative program reports as assigned or required; assists with the orientation and training of new staff; assists with the evaluation and continuous quality program improvements as assigned
Act as main liaison between Cochise County and ASHLine (Arizona Smokers Help Line)
Partner with healthcare organizations and work with interested hospitals to facilitate referrals and health systems change
Develop and implement a Smoke Free Campus or Wellness Policy
Facilitate and coordinate Retailer Education Programs

Required

Associate's Degree from an accredited college or university in health care, medical care, early childhood, special education, elementary education, secondary education, social work, nutrition, or closely related field OR two (2) years of related experience including health care, medical care, early childhood, special education, elementary education, secondary education, social work, nutrition, OR an equivalent combination of experience, education, and training which provides the desired knowledge, skills and abilities of this classification. Must possess and maintain a valid Arizona driver's license
Associate's Degree from an accredited college or university in health care, medical care, early childhood, special education, elementary education, secondary education, social work, nutrition, or closely related field PLUS one (1) year of related experience OR three (3) years of related experience including health care, medical care, early childhood, special education, elementary education, secondary education, social work, nutrition, OR an equivalent combination of experience, education, and training which provides the desired knowledge, skills and abilities of this classification. Must possess and maintain a valid Arizona driver's license
Bachelor's Degree from an accredited college or university in health care, medical care, early childhood, special education, elementary education, secondary education, social work, nutrition, or a related field from a regionally accredited institution of higher learning recognized by the US Department of Education. Must possess and maintain a valid Arizona driver's license and the ability to be covered by county insurance
