Responsibilities

Provide direct and supportive care to clients/patients, promoting their recovery and well-being

Monitor and document client/patient behavior, vital signs, and medical symptoms

Assist clients/patients with daily activities, such as personal care, medication administration, and transportation

Facilitate group education/structured activities, promoting social interaction and skill-building

Collaborate with the multidisciplinary team to develop and implement individualized treatment plans

Maintain accurate records and reports, ensuring compliance with regulatory requirements

Provide crisis intervention and support, as needed

Qualification

Required

High School education or equivalent

Two (2) years of responsible employment experience

Must be at least 21 years old to operate a company-owned vehicle

Valid driver's license in state of residency with an acceptable driving record

Current auto insurance

Ability to communicate effectively with clients/patients, families, and colleagues

Basic keyboarding skills and ability to navigate electronic medical records

Preferred

Experience in Human Services or a related field is preferred
