Rite of Passage is dedicated to improving the lives of youth and is currently hiring a Case Manager at Pompano Youth Academy. The Case Manager will be responsible for developing and facilitating case plans and treatment programs while collaborating with professional staff to meet the goals of each youth's treatment plan.

Responsibilities

Administrating, developing and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load
Collaborates with other professional staff to establish and meet the goals of each youth's treatment plan by providing individual counseling and skill development
Communicate with families, probation officers, referral agencies, etc. on the progress each youth on your case load
Provide an atmosphere that is supportive of the youth’s needs
Monitor and document clinical behaviors and activities
Participate and assist with educational, social, athletic and recreational activities

Qualification

Case management experienceBA/BS in related fieldIndividual counselingCollaboration skillsCommunication skills

Required

Possess a BA/BS in related field
Have prior case management experience, including three years' experience with juveniles
Be at least 21 years of age
Be able to pass a criminal background check, drug screen (we no longer test for THC for pre-employment), physical, and TB test
Be able to pass a search of the child abuse central registry
