FAMILY DEVELOPMENT COACH

SCO Family of Services is dedicated to supporting families and individuals in need, and they are seeking a Family Development Coach to provide case management and family counseling services. The role involves working closely with participants to develop Individual Service Plans, engaging with families, and organizing community events while ensuring effective communication with relevant departments.

Responsibilities

Reports directly to Senior Program Coordinator
Able to observe and assist during groups sessions at our secure detention facility 5 days a week
Provide case management services including creation and updates to Individual Service Plans (ISP)
Engage co-parents and children in the ISP process when applicable
Assist in recruitment efforts for program participation
Maintain regular contact with participants and ensure at least one goal is identified in their ISP
Document progress notes for all participant and family interactions
Complete intake forms, questionnaires, pre-surveys, and post-surveys
Track referrals and demographic information in spreadsheets
Provide family counseling services and educational support to children of incarcerated parents
Identify and refer clients and families to relevant community programs with a focus on DYCD initiatives
Organize quarterly community events for families and facilitate community-building activities for incarcerated fathers
Enter attendance and session data into DYCD Connect system (DOC PTS)
Collaborate with the Fatherhood Training Specialists, Young Adult Workforce program leadership, and other related personnel to provide comprehensive services
Ensure the integrity of the program, communicating with the Department of Youth & Community Development (DYCD) and the Department of Corrections (DOC) on jail and community-based program progress

35-Hour work week; Evening schedule (11:00 am to 7:00 pm)

Required

Minimum High school diploma or equivalent; Associate's degree in Social Work, Human Services, Criminal Justice or a related field, or equivalent experience preferred
Strong knowledge of community resources and challenges facing formerly incarcerated individuals
Ability to obtain Department of Corrections clearance and work within the constraints of facility guidelines. Including lock downs and personal screenings, this can be changed at the discretion of the Department of Correction
Excellent interpersonal, facilitation, and communication (written and verbal) skills
Good verbal and written communication skills required
Flexibility and dependability
Good organizational and time management skills, with the ability to work independently and under pressure
High levels of drive, determination, and commitment
Excellent teamwork skills
Excellent oral and written communication skills and a capacity to work with a wide range of people
Ability to act under own initiative and constantly strive to deliver excellence and creativity in your work
Ability to deal with complex issues and communicate them to management and staff members
Ability to prioritize urgent issues whilst achieving outcomes to agreed deadlines
