Responsibilities
Provides direct patient care, evaluates functional needs and outcomes, and establishes a plan of care. Conducts therapy practice within defined standards of care
Evaluate the effectiveness of treatment and modify as necessary
Assesses educational and cultural needs of the patient, family, caregiver, or significant other and communicates accordingly to achieve maximum care benefit
Documents evaluations, treatment goals, and plans following Hospital requirements and established guidelines
Regularly updates documentation and maintains prescriptions and signatures
Prepares discharge plans
Coordinates, delegates, and supervises responsibilities assigned to support staff
Coordinates with the interdisciplinary team on patient needs, treatment modifications, and progress
Attends and participates in the weekly team conference
May be required to work during inclement weather and other staffing emergencies
Provides a safe environment for patients, visitors, and staff
Assesses the risks for safety and implements appropriate precautions
Compliance with appropriate and approved safety and Infection Prevention standards
Maintains regular attendance and keeps up with assigned work schedule
Performs other duties as assigned to support the organization's overall effectiveness
Qualification
Required
1-2 years of direct patient physical therapy care and treatment experience is required
Virginia state licensure as a Physical Therapist is required/ compact state license
Basic Life Support/CPR Certification is required
Physical Therapy License (Required)
