Social Care Network Navigator
Buffalo Urban League is dedicated to enhancing access to care and promoting self-sufficiency for individuals and families. They are seeking a Social Care Network Navigator responsible for conducting screenings related to social determinants of health and providing navigation support to connect community members to resources and services.

Responsibilities
Conduct empathetic & culturally competent screenings for health-related social needs (HRSNs), in person, virtually, or by phone
Take referrals, follow up, verify eligibility, do intake documentation
Triage and route referrals to appropriate community-based organizations or providers
Maintain accurate and timely documentation of screenings, referrals, client interactions, and outcomes using the WNYICC IT / data platform
Provide follow-up support to ensure clients are connected to services, assist through the service enrollment or application process, adapt if there are barriers
Outreach & Community Engagement—informing the public about BUL programs, the waiver and eligibility
Sometimes travel within the region (Erie County and other parts of WNY) for community events, home or field visits
Qualification
Required
Associate degree in related field plus 2 years' experience, or a Bachelor's with at least some experience in human services / case management / resource navigation
Experience in case management, service coordination, or resource navigation
Ability to communicate well (written, verbal); comfortable with diverse populations
Administrative/documentation skills; attention to detail
Reliable transportation and flexibility to work outside normal office settings when needed
Ability to work evenings or weekends as needed
