Responsibilities
Conduct patient interviews, vitals, and health screenings under supervision
Assist with patient care including preventive, acute, and chronic condition management
Observe and support medication management and treatment planning
Collaborate with interdisciplinary teams including behavioral health, dental, and case management staff
Document patient encounters in the Electronic Health Record (EHR) in accordance with clinical standards
Participate in quality improvement and care coordination initiatives as appropriate
Qualification
Required
Currently enrolled in an accredited medical or allied health program (LPN, RN, NP, PA, MD/DO)
Seeking clinical rotation, externship, or internship for academic credit or licensure
Third Street must have an affiliate agreement on file with the school
Strong interpersonal skills and commitment to community health and health equity
Able to maintain professionalism and confidentiality in a clinical environment
BLS/CPR certification (as required by program)
Completes credentialing requirements within first 90-days of employment
