Home Specialist
Charity Care Group is dedicated to improving the lives of low-income seniors by enabling them to age safely and independently in their homes. As a Home Specialist for the HomeSafe program, you will connect seniors to resources, build a network of service providers, and ensure the program’s success through outreach and education efforts.

Responsibilities
Conduct outreach to licensed Occupational Therapists (OTs) and contractors to build a database of professionals interested in contracting with the HomeSafe program
Cold call, email, and engage potential service providers to gauge interest and qualifications
Maintain an organized database of interested vendors, ensuring all licensing and credential requirements are documented
Conduct physical in-home visits to assist seniors with application completion and eligibility verification for the HomeSafe program
Gather and organize required verification documents, such as income and age qualifications, to ensure compliance with HUD guidelines
Provide clear guidance to seniors and their families throughout the application process
Conduct door-to-door outreach in targeted communities to identify and engage eligible seniors for program participation
Educate seniors about the benefits of the HomeSafe program and assist with initial registration steps
Represent the HomeSafe program at community events, senior expos, and health fairs to promote awareness and participation
Distribute HUD-compliant educational materials, including multilingual resources, to underserved populations and individuals with Limited English Proficiency (LEP)
Collaborate with local organizations, such as Area Agencies on Aging, to maximize outreach and impact
Ensure all activities adhere to HUD’s 24 CFR Part 58 requirements and other relevant regulations
Contribute to biweekly progress reports detailing outreach efforts, client registrations, vendor recruitment, and program outcomes
Qualification
Required
High school diploma or equivalent (Bachelor's degree preferred)
Experience in outreach, social work, customer service, or vendor sourcing is a plus
Strong organizational and time management abilities
Excellent communication and interpersonal skills, especially when engaging with seniors and underserved populations
Proficiency in data entry, maintaining accurate records, and building professional networks
A compassionate and empathetic approach to working with seniors
Patience, professionalism, and a commitment to improving lives
Preferred
Bachelor's degree preferred
Experience in outreach, social work, customer service, or vendor sourcing is a plus
