InternMontefiore Einstein Technology is a part of Montefiore Medical Center, and they are seeking volunteers to assist with social determinant of health screenings. The role involves screening patients for various needs and providing outreach to ensure patients are scheduled for clinic visits.

Responsibilities
Volunteers will be given a patient questionaire to screen patients for gaps in social determinant of health screening (transportation, food insecurity, housing instability, etc)
The volunteer will then give this screen to the physician conducting the visit
Volunteers will provide outreach to patients
Volunteers will be given patient phone numbers (for patients lost to follow-up) to inquire if the patient would like to be scheduled in clinic
If the patient is already scheduled then the volunteer will be given the patient phone number to call in order to confirm the appointment
