Avista Senior Living is currently looking for Certified Caregivers and Med Techs to join their team at the North Mountain community in Phoenix. The role involves providing care and support to residents, administering medication, and ensuring a safe and clean environment while promoting independence and self-care.

Responsibilities

Read and implement care/activities of daily living (ADLs) according to the resident service plan

PASS MEDICATION according to physician orders if applying for the med tech position using the electronic health record

Respond to resident and other team member needs in a timely manner

Prioritize communication between other team members using group messaging and two-way radios

Demonstrates empathy and develops caring rapport with residents and other employees

Willing to support and lift other team members when they are struggling or learning a new process

Promotes resident independence and self-care

Delivers and documents resident care and services in accordance with individual service plans

Maintains accurate and complete records of delivered care in the electronic health record

Completes incident reports sensitively, accurately, and timely

Understands safety and infection control processes and demonstrates practices to control the spread of communicable illnesses

Promotes a clean, orderly, and safe environment including performing housekeeping, when needed

Recognize, respond, and communicate with the maintenance team to keep the community, work environment, and resident apartment in a clean, comfortable, and safe condition

Adopt a code of conduct in how we behave with team members, visitors, vendors, and others in accordance with Company values
